Format numeric field in forms

I have attached a sample Database with the field formatted.
There are other ways of doing things but this is what I would do.
The attached is not for decimals, only whole numbers.

Just one more thing. Regardless of how the number is formatted it is stored in its proper state. e.g. 0012 will be stored as 12.
