How do I design/create a form without using data from a table or query and have the page header and footer added?

Hello dejany!
In "database window" click on "Forms" tab,
click on "New", select "Design view",
don't shoose a table or query,
click on "OK".
For Header and Footer click on "View",
click on "Form Header/Footer.
