Help with Form please

Hi, After reviewing the forum and tutorials I am still at a loss. I have a form where I have 3 txtboxes for input and one list box where I want to be able to select from the list box and send only the selection to report. I also need a search button to requery and button to send the selection to report. I tried to put a button for search but it is giving me an error. I am going to attach the dummy db that I created for someone to please review and let me know what I am doing wrong. thank you. Ooops not sure how to attach the db.

Thank you for the information. Attached is the dummy db. I would very much appreciate how to select the ids in the listbox and only send these to report. Thank you.
