Help with tick box

can somebody plaese help me! i need to use tick boxes on forms named orders, enquiry, booking in , job shet, time sheet and finaly gone to invoice. i need all of these forms firstly when filled in to place a tick automatically in the relevant box and then a new form needs to be made to display the stage which each contract is at ie, enquiry, job sheet gone to invoice. if anyone can help i would be very gratefull.
 
Well either a field on the top/main table with a status (uses a lookup to status's held in a lookup table) or you have multiple fields on the main table as boolean/tick boxes.
