Login Form -n- More

This is a two part Request...

Part 1...

I want to create a form that prompts for a user name and a password. Can someone
help me get going in the right direction on this?


Part 2...

I am trying to build a prospect/contact database. I would like everybody to
use the same database to enter thier contacts, appointments and, followup info.
I also want them to use username/password to login, however when they login
I only want the contacts, appointments, etc... of the username/password entered
to be opened... Does anybody know if this is possible... And if you do...could
you help me with this as well...
