Here is the file. Per the email:

The form in question is listed under the Ineos Form
Once the Forms opens up, Click on the Year to Date, This is the form, I am trying to make it work with the combo button like the other tabs.

I'm catching up from being out of town, may not be able to try and digest the data for a bit.
