Need help with forms (1 Viewer)

Hi

I need advice on how to create form(s) for the attached relationships.

I had no problem creating a form (with subforms) for a series of 4 tables one-to-many relationships (tbl_replicate, tbl_tcpbsdata_lm, tbl_tcpbssiteply_lm, tbl_tcpbssite_lm).

But incorportating tbl_activty_site and tbl_activity has me stumped! can anyone give me a hand?
