New database question--HELP!!!

I have a database with two tables. I am trying to create a type of items check out database. In one table are my videos. The other table contains everyone who can check out the videos. I would like a form to display, where I can choose a video, then an account number and then the account number holder's information would appear. I think this is probably easy, but I just can get a handle on how to create. If anyone can help I would appreciate it. Also, if there is a sample database like this somewhere, please let me know how to find it.

You also need, at the very least, a third transaction table to hold the data on who borrowed what video (and probably what date they borrowed it).
Then simply use a form with combo boxes to populate this table.
The next stage will be to identify what videos are out, so exclude them from the query feeding the video combo.
Not too difficult and should provide some fun along the way.
