number and date = new record

Pls help..

I have a table and i want to enter a new record but it has not to be repeated by date..

example: record no is 1, and the date is November 15,2007..

i just want to use the record no again next year...

how can i possibly do that?

how can i make a value of my record to increment...
example: from record 1 it will automatically appear in my form is record 2 and so on and so fort..

but on the following year it will back to 1 in order to begin another record...

im totally dont know what to do... is that possible?
