Pass value to Query

Hi I have attached a sample DB of what I want done. If you open the DB, you'll see a form called "Form1." If you click on "Code 1" that should pass a value of 1 to the criteria of the query. If you click on "Code 2" that should pass a value of 2 to the criteria of the query. If you click on "All" it should pick up all values in the query. How would I do that?
