Patient Attendance Database - Dates specific for one patient shows up for all of them

Hi guys,

I am trying to create a database for a clinic, and am severely stuck on how to input appointment dates for individual patients.
I have been using the 'student' database from office.com as a template for how to save the dates (given that appointments and attendance are exactly the same!), however, even after following what has been set up in the 'student' template database, I can't seem to replicate it.

Every time I add multiple visit dates for a specific patient, these exact dates show up for every other patient in the database. I need to be able to add different dates for all the different patients.

I am completly new to Access, so any simple help would be much appreciated!

Unless the Student Database Template is based on scheduling students for classes, it is not going to accomplish what you're looking for.

I'm attaching an incredibly simplistic database that should at least give you an idea of one way to accomplish this. Of course, if you were to use this database, you will still have a lot of work to do in adding additional fields to all of the tables to capture the information you want to capture, updating the forms, prettying it up, etc.

Start off looking at frmAppointments to get an idea of what you can do, and then look though each of the tables and forms to learn more about how each piece works.
