Pivot of two data sets with Count of Yes and No

Dear all,
Greetings

I need help in creating a pivot having 25 Districts and 36 Questions with answer of Yes or No for each District. (the same questions data will come again and again on different dates.

I need pivot with questions/ district on either side and then in the values i need count of Yes and Count of No.


Can you please help
