Prepare Data for PDF Applications

Good afternoon,
I am working on a project involving pdf applications that users will need to complete. To help them save time, I want to pre-populate it with some of the information in my database. However, I need to flatten some of the data, but I'm not sure how. For example, a have a field for education, but there could be many education records for one user. Another question is, can I flatten this table and string others like it so I end up with one long data record. I want to use this table to "code" the pdf applications, then process it similar to a mail merge. The field names in the resulting table (or output to Excel) would need to be the same as the applications' fields.
Your help is greatly appreciated.

To be honest, I am not a coder. I can create queries in Access and that's about it. I'm sorry. Thank you for your help.
