Pulling an input box value from a form into a report

I have a form with a button on it that opens a report. However, i would like to be able to specify the title of the report when i click on the button. What si the best way to do this? What i am trying to do now is add an input box to the command button that opens the report. I get the input box to work but i can not figure out how to pull the text into the report. Thank you for your help...
