Query picking up more than one record, under unique identifier?

Hallo guy's and gals.,
Im in need of some desperate help, I have gone and completely broken this database........
Where am I going wrong.
All I want, is when you click the next section button on the from, the next section form should open up, one should be able to fill in the information in a new blank form, that is related to the previous form, and move on to the next, repeating the process. Start at section one fill it in hit next, the next section pops open (blank) with a relation to the previous on and so on and so forth. Surely this can not be that difficult. Im getting all sorts of problems......
now I can even get the relationships to work properly
