Query Won't Generate Report

I have a Form >>> Query >>> Report setup and when I run the query it won't populate my Report and display that to the user, rather it just shows its' own query output. What do I need to do to have the form pass the criteria to the query and the query to pass the appropriate data to the report and the report to display as a preview?
