Does anyone have or know of a questionnaire database example???
I have been playing about with my own effort but so far I cant seem to get the questionnaire form working without alot of end user work. I have tables set up in a way which i think is correct as it will allow me to query the results easily (produce reports on yes/no answers).
I would like the user to open a form, enter the case no and then just give a yes/no answer to the questions. I am not sure how to get the questions to appear on the form without the user having to select them manually one by one from a combo box.
My form attempt included a form, with a control source of tblcase, and then a subform with control source tblMain, linked by case no. On the subform I had 3 controls case no, a combo box for questionid which dispalyed the full question using tblQuestions and then a check box. Viewing my attachment will explain this all. This is working but as I mentioned above this involves alot of end user work and as the questionnaire will contain 70+ questions. This method also doesnt ensure that all questions will be answered.
