Question Questionnaire recording troubles and additional problem

Hello!
So i have been reading and reading as much as i could regarding how to make a database which would enable me to record the answers i recieve back from patients for a research project..Later on i'll have to worry about the analyses but right now i just want to figure out someway i can put the questionnaire in access so that i may electronically record all the answers i recieve back from each patient. Below is an example of one simple questionnaire where there is one answer they may choose from each heading (ie: for mobility they could only choose one option like 'i have no problems walking about')....I was given a template to follow but when i start adding things to it, an error message pops up saying "cannot add any more controls on this form"--attached is a pic of the design view of that template..Is there some way i can add more tabs on top so i can add more questionnaires (basically i have 4 groups of the same questionnaires i need to record--one for baseline, than 3 months, 6months, and 12 months)...If there is no way i can add more tabs can I link another form to it??

It looks like you have a series of tabs on your form with many controls on each tab. collectively you have exceed the normal limit. Try placing the questions for each tab on a seperate subform and inser the suborms on to each tab. This may aleviate the issue. However you may need to rebuild the form from scratch as Access remembers how many controls you previoulsy had on the form.
 
Thanks so much for the prompt reply! As this is a template and i am a total newbie at access i unfortunately don't know how to insert a questionnaire into access so i may record the results i obtain..In the example i had above of the simple questionnaire, how may i insert that into a table so that i may create a form that i can customize which would resemble that questionnaire?

Thanks so much for the help, i've been going nuts over this!
