Report Dates Problem

Everything works, except I cannot get the Start date and End Date to show up on the Report.

I created a Form called:Input Form It contains two text boxes- Enter Start Date: and Enter End Date: for Labels and two unbounded Text boxes.
I placed the following into the Initial Date Field in the Query Criteria space: Between [Forms]![Input Form]![StartDate:] And [Forms]![Input Form]![EndDate:]
The Report prompts me for the dates and runs correctly. However, the Text Box, in the Report Header, fails to capture those inputed dates. I placed: ="From "+Format([Enter Start Date: ],"mm","/","d d","/","yyyy")+" to "+Format([Enter End Date:],"mm", "/","dd","/","yyyy") to place the date interval on the Report, but NO SOAP!. These Dates do not show up as I would like them to do. Instead, I get a copy of the command or #Name? What is missing here? Something trival I'll wager!

If the date fields on the form are populated click a button that runs the report
create a field in your report header then in the properties,control source place the following code
="Date Range Selected " & Forms!YourForm!YourStartDatefield & " To " & Forms!YourForm!YourEndDatefield

This is no big deal, just a matter of syntax and execution.
This is what I did:
1.Created Input Form with two unbounded fields
2. Named Text4 Box to Enter Start Date:
3. Named Text6 Box to Enter End Date:
4. Created two Command Buttons ( I'll only describe one) as follows:
First Button Name OK and the second (Close) to exit the form
1. When button is inserted dialog Box appears with two columns:
Categories and Action
2. Chose Categry- Form and Action Open a Form
3. Chose Show all records - though the specfic seemed better. (neither worked.)
4.Text Name for Button OK
5. Meaningful name "Open form"
saved and exited.
6. Went to report and placed what you suggested:
="Date Range Selected: " & Forms!Input Form!StartDate & " To " & Forms!Input Form!EndDate(Text4 and text6 substitutions did not work, also
It gave me a syntax error.
Tried ="From "+Format([Enter Start Date:],"ddd"", ""mmmm d"", ""yyyy")+" to "+Format([Enter End Date:],"ddd"", ""mmmm d"", ""yyyy")
Still no good.
I cut out one button "Close Form" and still-No Soap!

I am utterly amazed at how something so trival, in FoxPro etc., is so onerous in Access. The more difficult things it (Access) does well-the simple-that's another story. I am new to this program and I dread more simple stuff that lies ahead.

Report Problem

Here is the Paradigm:

1.Created Input Form and saved it
2. Set Query to reference Input form:Between [Forms]![Input Form]![Start Date:] And [Forms]![Input Form]![End Date:] Note:EndDate and Startdate are from Input Form button controls.
3. added two controls to Input form: Open & Close
Report still demands double entry (putting in the date range twice to work correctly.)

Checked "Event" On Click, Command button Open, and observed the following Code:

The problem, I think, lies in the code, which I do not know.
What do you think? What an onerous way to pass variables!!
