Report from Continuous form

Hello I am trying to generate a report from a Continuous Form but the report is only pulling up one record instead of the 9 that are displayed. Any suggestions?

How are you opening the report? What is its record source? What determines what records appear on the form? Within those answers is your solution.

the report should be based on a query (same as the form)
if the query pulls 9 records, so will the report.

It isn't, I am using the filter option on the form say Tax id or Last Name. If use Tax ID there are 9 records but when use the table that is used for the form and then use [forms].[Any Name form].[tax id] and [forms].[Any Name form].[last name]in the criteria field from the table, just one record comes on the report although the 9 records on the form with distinct last name and address, still only one record appears on the report.
