Report Question

I want to create a report that looks exactly like one of my customer’s forms.

I have created the table and data input form, now is it possible to create a report that would include my option list results and the check boxes too.

See attached screen shot for example.

Create the report and open it in design view.

You will have to manually move the fields around to resemble the supplied form.
It will take some time and you'll never get it exactly the same but you can come close.

Hold off til Monday before you start. Someone else may have a better idea.

I don't have an issue creating the report, the issue is how do you create the report so it has the check boxes and the when you choose the report it open up with the item checked you selected from the input form.

When I went to create the report none of my check box option groups were created as check boxes. I can not figure out how do it.

Open the report in design view,
Right click the box you wish to be a check box,
Select "Change to"
Select check box

Report With Option Check Boxes

Can some one please look at what I'm trying to do and see if they can help?

I want to take all the information on the data entry form and create a report that I can print based on the "ID" number and have the same check boxes on the report and the ones selected for that record.

When I create the report none the check box fields transfer over as check boxes. I have tried to create the option list on the report but can't get them to tie back to the record.

Example: The form field “Reason For Submission” is an option group with 10 options that are stored in the table as a number. I want my report to show all 10 of the option heck boxes and show which one is checked based on what was checked in the entry form.
