reports not showing on click date

Dear friends

i created patient report data like date wise then i createdform for view the reports date. After opened form if i clik one date particular date report is opening but on click another date is showing first time clicked data report. At the same time if i closed and open the form is coming each and every time i closing and opening the from this is very painfull. Anybody help me to open the report by clicking dates without closing and opening form.

Do you close the report when you choose a new date?
Show your code that open the report after clicking the date!
