there could be multiple actions at different dates till the matter is closed out
Sounds like you will need a table inbetween the two tables that way you could add a new event type with a date of event if you don't want them to add dunplicate Actions you could use a combination key arrangement.

I have found some example of incident reports see attached. Regarding actions you can use a query to make a subform and it would then be coded to be linked to the main form at all times.

I have not worked this out yet how it is done, maybe someone out there knows how

I've attached a small example database which shows the Incident Form and Actions etc, as you can do this with only a few tables but use queries
