save 5 last status and show only the latest in a form

i have table with fields name,phone and status1,status2... status5
i have a form to add records which uses status1.
now i want to build a form to update status... it should how many fields for status i have used.. and allow me to fill the new one...

how can i do so.
plz help..

Hi,

I think you ought to reconsider your table setup because from the fields which you mentioned, it is clear that your table is not normalized. Having several status field in your table is not the way to go. Maybe if you explain exactly what you are trying to achieve, someone will help you set up your table(s) appropriately.

for every unique person i need to have at max last five status. for example.. if we call a new person.. he says call me on wednesday--- his staus1 is "call on wed" on wed we call hom again and he says call on thursday----status2 is "call on thur" something like that.. i dunno how to normalize a table..
