selections on checkbox / options

Hi,
I am not an experience user, at present, I face two problems in one of the form
1. Display format in a continue form, please see further down for required display format.
2. How can I allow multiple checkbox or option selection in a continue form.

Since I don’t know how to do the required display format, Present display as
Toplevel sublevel item
Picture Large 108 x 96
picture Large 200 x 200
Picture Large 300 x 300
Picture Medium 96 x 72
Picture Medium 48 x 36
Picture small 8 x 8
picture small 10 x 10
frame Wood
frame Wood effect
frame pastic
chair Wood black
chair wood red
chair plastic black
chair plastic pink

Required display format

Picture (non selectable)
Large (title – non selectable)
108 x 96
200 x 200
300 x 300
Medium (non selectable)
96 x 72
48 x 36
Small ( title – non selectable)
8 x 8
10 x 10
Frame (non – selectable)
Wood
Wood effect
Plastic

For the second problem, I have tried checkbox as well as options. Please note this is a continue form. When I select one of item along the list, all boxes or option boxes were checked. What have I done wrong. I didn’t use any code, just use the form wizard to create the form and add a checkbox or option along the display. Is there some settings which I am missing ? Also I do want to select more then one item. e.g. from the picture - large 200 x 200 and small 8 x 8 and 10 x 10 then Frame - wood effect etc.

Any help appreciated
 
This is normal behavior for continuous forms when your checkboxes aren't bound to an underlying field in your table/query. If the checkboxes were bound this wouldn't happen.What exactly are you trying to do with these checkboxes?

Thank you for replying.

I got a user who have all these items grouped in 3 levels, 2 of them are titles and the 3rd one is various items which related to each project. The titles she uses was very long and I simplified them in my original posting.

Since the format was used for over a long period of time and don't want to impact the layout of the past document.

OK what I want or she wants is when there is a project, those items or products display in the required format and she can just tick the box of items which apply to the project.

For example,
project A would required large picture of 300 x 300, frame of wood effect, chair - wood in pink

Project B would required small picture 10 x 10, medium picture 96 x 48, chair - plastic in black etc

Of course the products list is much longer then my example

Hope I have made what I want to achieve clear otherwise, please do get back to me

Also what do you mean by bounding, do I do it in the table or on the form please?
