Table not recieving data

I have a table I'm trying to poplate from data input from a form. On the form, I've inserted a drop down box with a list of options to choose from. The chosen data is NOT populating into the table though. I tried to change it - but have managed only to replicate the drop down box in the table. WHICH is of course not right! HELP!!!

Is the form bound to the table?
