Update query

I have application that inserts a record into an Acces table everytime a yes/no radio button is clicked:

WC_Memo:= "Flexible"
sqlstring:= INTO Manvol_1_4 (Title,WC_Value, WC_Memo, Dateofentry, Timeofentry)VALUES('"^title_vol^"',"^volunteer^", '"^title1^"','"^Date^"', '"^FullTime^"');"

My question in the application has 4 parts:
Volunteer (Yes/No)
Organisation (Yes/No)
Beneficiary (Yes/No)
Community (Yes/No)
I would like to update the table or even create a query so only the last radio button 'insert record' is displayed. for each of these parts- Im stuck

Are you talking about hiding the three other button on a form?

OR

Create a query with only one field?

I need four 48 fields in the query to show as their are 12 questions each with four responses.
The four responses for each question are either volunteer/organisation/beneficiary/community.
I need to show the latest entry for each of these responses
