Using a Form to Build Query Criteria (1 Viewer)

I have been searching the internet for an example of how to make a form that I can use to select query criteria from. For example:

Form

Select accounts with "Gas" or "Elec" or "Water"

In a drop down box

then maybe have two fields with date from and to perios for the active dates.

Any help or examples will be appreciated
