weird append query results

I want to append a row with a specific selected facilty name

This is my append query. no matter what facility_name i use it appends that record 13 times. it probally is something stupid but i have been looking at it for hours and can't seem to find my error

INSERT INTO TrackChanges ( [User], [Date] )
SELECT CurrentUser() AS Expr1, Now() AS Expr2, Data.*
FROM TrackChanges, Data
WHERE (((Data.[Facility Name])=[Forms]![FrmChange2]![Facility_Name]));

You are getting 13 rows returned from your select. Run the select by itself to see.

I ran it as a select query and it still spit out 13 rows...
but there is still only one record with the specified facility name

I appreciate all your help. pat your advice did the trick. such a random thing for access to just spit out 13 identical records
