Add Pictures to My Records

Hello
Im working with a employee form.
I have Name, Last Name and employee number fields on this form. I want to add a option for add the employee picture. Add / Change or Delete the picture. How can I do this?

I want to see the employee image and his information. I can save all images into a folder.
