Another Monthly Report Question

I have found many posts on this subject, but can't find the answer I'm looking for. I would like to create a form requiring user input to generate a monthly report. I have used the calendars often and they work great, but, what I would really like this time is just a form where the user would select a month/year from something like a list box to generate the report. To take it one step further, I would like only the months/years for which records exist to display. I would really appreciate ti if anyone can help with this.
PS. All dates are formatted in yyyy/mm/dd, Canadian hospital standard.

I apologize if I'm a little thick. Does that not require manual input to add a new record (month) in the table every time there is data entered for that month? I was looking for code to create the list.
