Question Downtime Database

Hi all.

What would be the best way to go about this:
I want a database for employees to use.
Employees select there name from a dropdown. Then a week number I have been trying to get the form to default the current week number but its proving difficult.
Saturday to Saturday weeks.
They go to a day e.g Monday put in a figure (time) and a reason for this downtime and save.
Each day can have multiple downtime reasons.

Is there any sample databases that follow that kinda unclear statement above

Hi! - Probably the best thing to do is not to worry about the Sat-Sat weeks when users are entering data, just worry about that when you're reporting. Just have the user select their name, the date, and enter a downtime reason. Then when you're reporting, group by the week you want?
