Googlemaps form

Hi Guys!

I created a form based on query that shows customer information. Now, i want to create embeded googlemaps object which will show directions from place A to place B where place A is place of my company and place B is City text box from my form.

Basically, i want form to show me a directions from my Company to the Customer.

Is it possible?
