How Do I duplicate a form with same Data

I have a form that i fill out for each service order. This from should contain one control # that is automatically assigned for each record. How can i make a duplicate form with the same data and then assign new control #. Sometimes i have to assign many control numbers for the same service, so i need to keep the same form data but change the control number.
