How to use a questions table for the text in a questionnaire

Good day,

I am developing a questionnaire. It is my first one. I decided to have all the questions in a table with a q_ID identifier. for a given set of questions I want to gather record/data on the person that answer the questions and his answers. I am battling to understand how to present all the questions on one form record where the form record is related to the person answering the questions and secondly, how to keep the questions the same and visible when moving on to the next record.

Will appreciate some advice here.

I suggest taking a look at what's already out there. For example I googled "ms access questionnaire database design" and came across this. If it doesn't do what you want it will give you some ideas about how you might structure your database.
