Increment Text box value from combo box selection

Is it possible to assign token number for each patient from the doctor who they wish to consult????

Doc Token#
----- -----
Doc A token 1
Doc B token 1
Doc A token 2
Doc B token 2
Doc C token 1
Place a text box to display token number. if you want you can put any number on that box but when you select doctor name from combo box the number for that doctor will count from the last token# for that doctor...

any ideas pls hlp..
