I've had a lot of experiences creating databases but a questionnaire database is the most complicated I've had to create. I'm pretty close to finishing but I'd like to hear some comments if I'm doing something wrong. So if you agree with my design there's no need to tell me (unless you want to). This is a survey given when an employee leaves the company, if that helps anything.
My requirements (or my ideal design) are that an employee can take the same test or different tests several times, depending on how many times they're hired and quit/fired. A question can have a multiple choice answer, a free response answer, or both. A question or series of questions can be preceeded by a common heading. Also, a question can have a subquestion (a question that can only be asked after another specific question). I want to be able to enter data the exact same way the employees' filled out the test (same questions, same order). Once the responses are entered I need to be able to redisplay the survey exactly as it was answered.
Does my database provide for all of these things?
