Report for multiple blank fields

I have created a report with a query attached that basicaly shows every record in Surname order. What I would like to be able to do is only show the records where there is a blank field/fields.

* Date 1, Date 2, Date 5 could be filled out while Date 3 and Date 4 blank
* Date 2, Date 5 could be filled out while Date 1, Date 3, Date 4 bank
* All Date fields could be black or any 1 field or multiply fields.


Name - Joe
Surname - Bloggs
Date 1 - could be blank
Date 2 - could be blank
Date 3 - could be blank
Date 4 - could be blank
Date 5 - could be blank

SELECT ...
FROM ...
WHERE Date1 Is Null OR Date2 Is Null OR ...

Where would I put the code and what do I do with the select and from commands?

Could you please provide an example bit of code that I would be able to modify.
