The form field will show when check will be yes

I have a table here some field and i create a form.
in my form i have these fields
sl,field1,field2,field3,field4,field5,field6,field7,field8
and one submit button
and two check box

Plan Descriptions
Generally when i'll open my form it will show just SL , two check boxs and submit button.
but when i'll check yes on checkbox1 it'll show field1 to field4 with submit button. this time field5 and field6 will hide.

when i'll check yes on checkbox2 it i'll show field4,field5 with submit button. this time field1 to field4 will hide.

Shortcut of plan
If checkbox1=yes then
show
Sl,Checkbox1,field1,field2,field3,field4,submit button

If checkbox2=yes then
show
Sl,Checkbox2,field4,field5,submit button

Please any one help my to this task.
