writing queries to retrive data in particular combinatio

I am a little new to writing queries. Could someone please tell me how should I write a query so as to display the data. The participant ID should be input by the user and then I want to lookup the records based on this participant ID.

Second this I want to do is, I want the user to enter the Study ID, Measurement ID, Value ID, Iteration ID. Depending on these 5 values I want to search data in a table in the combination entered by the user. This combination will find a particular value in the table and return it to the user. could someone please help me out in how to do this?? Help Please !!!!
