Yes or No!

I'm using Access 2000 and the yes/no combo box is bound to a table. Like i say, i can see fundamentally what the problem is now, my the subform is a set range of questions on each record, but the way i had set my table up it's treating it as one continuous record - i just don't know how to go about the alternative.
 
I attached a screenshot of the form in an earlier post. It might be best to take a look at that because i'm thinking what I'd like to do must be fairly simple (just not for idiots like myself).
There are two parts to the form - at the top there are basic details to fill out regarding the circumstances of the inspection (where, when, who), then there is going to be a set of standard questions, against which the entree must answer yes, no, n/a and record any notes/comments.

I've been setting this up as a form with a subform on it (containing the questions) - is this where i'm going wrong? Obviously, i will need to run queries and reports on responses to these questions so they each record has to be recorded individually.....
 
Ok attached as 2000 format and actually put some data and correct tables in this time so it works :o
