Complicated request from client

Hello brilliant minds!

I need some ideas for a customer satisfaction survey that includes the employees that worked the job and their pictures. Customers are to rate each employee in several areas. In addition to some basic yes/no questions about the company in general. I am stuck in my own head - I know how to pull the data from their access database, I had thought about using Outlook with it, but found out that's no longer an option. Any ideas?

Ideas about what? Hard to guess what you need but cannot handle yourself so be specific.

Sorry I wasn't clear. What tool or language do I need to create the interactive customer satisfaction survey that has to include pictures of the employees that worked on the job?
