What would be the best way to capture the attached "Customer Satisfaction Survey" Access, Excel etc.

The plan is to take this survey once a month over the phone and provide a report about the outcome to the president.

If that's all you need, then I would say in Access, a simple table with one input form and a couple reports reports would do nicely! The upside to Access would be the input form and the custom push button reports you could do rather quickly...
 
If this is their first db, would you really subject them to all that normalization stuff? Looked to me like a single table would work fine...

And if they were only doing several surveys a week or month, seems Excel would work fine...
 
Danny, Do you just need a simple means to manage the results of this one survey or do you want to put the time and effort into doing a system that will allow you to do other surveys?

I'm glad you ask. Actually, I should put some time and effort into designing a system that can be used to capture other surveys. The reason is that we have 4 departments (each with diffrent survey questions) and I was asked to design for now just for one department (as a pilot) to see the result. I'm sure that I'll be asked to do the same for other departments in the future. And, my goal is to get as much information as I could from the forum (how many tables, relationships, queries etc.) I need to create this survey database so I can use the same method for the other 3.

In my opinion the answer is a resounding YES!!! A normalized database will pay dividends down the road. A survey database has a particular type of structure that will aid in entering and analyzing answers.

In that case, I recomend you search the form on 'Survey' and see how the really good ones are done, making sure you get the hang of normalization...

If you get hung up on a concept, post back, there are some really good db'rs here!

The attached db is a simple survey dbase I just knocked up.

You will have to add validation to the fields and Buttons.

Open the Form and fill in the fields click save record record and your selections will appear in the table Tab_Survey

A survey app should have a structure like this:

tblQuestions
QuestionID (PK Autonumber)
Questiontext

tblRespondent
RespondentID (PK Autonumber)
Firstname
Lastname
other info about respondent

tblResponses
ResponseID (PK Autonumber)
RespondentID (FK)
QuestionID (FK)
Response

That's a bare minimum. Depending on the strucutre of the surveys you might add

tblAnswers
AnswerID (PK Autonumber)
QuestionID
Answer

This will predefine answers to the questions. Answers are then displayed usinng combos that query tblAnswers.

If respondents will take the same survey multiple times, then you might have add a datestamp to tblResponses or a parent table that deals with that survey.

The main point here is that each question is a record in a table and each response to a question is a record in another table that identifies the respondent, the question and the response.
 
That's a good start, but there's still a lot missing :)
 
Like I said that's bare minumum. but at least it shows the correct direction.
