Group By Month and Year as well as Count instances

In my table i equipment dates for the last 5 years failure. I would like to create a query that groups the dates by month and year and then counts the number of failures in that period (i.e. 01/2006 57, 02/2006 66, 03/2006 90 etc)

You haven't said what your problem is so I will assume that it is not the criteria nor the Count but the grouping, as well as Grouping on Format(yourdatefield,"mm/yyyy") you need to first Group on Year(yourdatefield) but you do not need to Select this so uncheck Show.
