Need help with reports (1 Viewer)

Hey guys ,
Problem 1: I need to make this database , which trainees are gonna fill out .
This DB will also 10 questions , with 4 options. ( stongly agree, agree, disagree , strongly disagree ), So as you can see you have text , but i want a value to be associated with the field.

Problem 2: I want some questions to have less wieghtage than others and some more..
however the maximum total for questions should be 100.

Problem 3,: i need all this info to reflect on a report ...along with details like Training batch name , trainer's name , date . ..The report is also gonna show the total points scored .

Sounds like an excellent example of when to use an option group.
